The IDAHO CITATION AND COMPLAINT (Form ID-17-5R) is
a five-part citation. (Price
Group 16) The face of all five parts is shown at right.
The backs of the first and second copies contain court docket
information. The back of the third copy ("Defendant's
Copy") has instructions for the violator to follow. The
back of the fourth copy ("Officer's Copy) contains Officer's
Notes for Testifying in Court. The back of the fifth copy
("Driver's Services Copy") contains the Idaho Uniform
Citation Abstract of Judgement.
The citation is 5.5 inches by 8.5 inches after being torn
at the perforations and is booked in sets of twenty-five.
It has space for imprinting your Department Name, Judicial
District, County and your Court Name and Address. As with
all our tickets, there is no extra charge for consecutive
numbering and up to five lines of crash imprinting. Stock
