WEST VIRGINIA CITATION AND COMPLAINT (Form WV-17-4)
is a four-part citation. (Price
Group 16) The face of all four parts is shown at right.
The backs of the first two copies contain an Abstract of
Court Record. The back of the third copy ("Officer's
Copy") contains space for Officer's Notes and diagrams.
The back of the fourth copy ("Summons") contains
instructions for the offender to follow.
The citation is 4.25 inches by 8.5 inches after being torn
at the perforation and is booked in sets of twenty-five. As
with all our tickets, there is no extra charge for consecutive
numbering and up to five lines of crash imprinting. Stock
tickets are shipped within seven business days from
time the order is received.
