WASHINGTON INFRACTION CITATION (Form WA-17-4I) is
a four-part citation. (Price
Group 16) The face of all four parts is shown at right.
The back of the first copy ("Court Copy) contains spaces
for Officer's Notes. The back of the second copy ("DOL
Copy") is blank. The back of the third copy ("Defendant
Copy") contains instructions for the offender to follow.
The back of the fourth copy ("LEA Copy") contains
the same backer as the Court Copy.
WASHINGTON CRIMINAL CITATION (Form WA-17-4C) (Price
Group 16) is almost identical to the Infraction Citation
except the Gold Defendent Copy contains different instructions
on the back for the offender to follow.
Both citations are 5.75 inches by 9.5 inches after being
torn at the perforation and are booked in sets of twenty-five.
As with all our tickets, there is no extra charge for consecutive
numbering and up to five lines of crash imprinting. Stock
