VIRGINIA SUMMONS (Form VA-17-5) is a five-part citation.

Group 17v) The face of the Court Copy is shown at right.
Page 2 is the DMV Copy and contains information on the status
of the offender's drivers license. Page 3 is the Defendant's
Copy and contains a waiver of trial and payment instructions.
On the back it contains rights and obligations of the offender.
Page 4 is the Agency copy and has spaces to fill in the Case
Disposition. Page 5 is a duplicate of Page 4 but is on green
paper. On the back it contains code numbers and abbreviations.
The citation is 11 inches by 8.5 inches after being torn
at the perforation and is booked in sets of twenty-five. As
with all our tickets, there is no extra charge for consecutive
numbering and up to five lines of crash imprinting. Stock
tickets are shipped within seven business days from
time the order is received.
