OHIO CITATION AND COMPLAINT (Form OH-17-5) is a five-part
carbonless Ohio Multi-Count Uniform Traffic Ticket (MUTT) for Ohio Police, Ohio Sheriff and Law Enforcement. (Price
Group 16) The face of all five copies is shown at right.
The back of the first copy (white "Court Record ")
contains spaces to put docket information, attorneys, fines
levied, etc. The back of the second copy (white "Abstract
of Court Record") is blank. The back of the third copy
(blue "Defendant Copy") contains instructions for
the defendant. The back of the fourth copy (pink "Agency
Record") contains spaces for Report of Action on Case,
Officer's Notes, and Arrest Notification. The back of the fifth copy (manila "Agency
Record") contains spaces for Report of Action on Case,
Officer's Notes, and Arrest Notification.
There is no extra charge for consecutive
numbering and up to five lines of custom front side text . Stock
