NEBRASKA CITATION AND COMPLAINT (Form NE-17-4) is
a four-part citation. (Price
Group 19A) The face of all four copies is shown at right.
The citations are pre-numbered and bar-coded.
The back of the first copy (white "Complaint")
contains the Court Record and spaces to enter pleas, fines,
etc. The back of the second copy (canary "Officer's Copy")
contains spaces for the officer to fill in road conditions,
clocked speeds, etc. The back of the third copy (blue "Prosecutor's
Copy") contains information on the disposition of the
case. The back of the fourth copy (hard "Defendant's
Copy") contains waiver and plea instructions.
The citation is approximately 4.5 inches by 8.75 inches before
being torn at the perforation and is booked in sets of twenty-five.
Each book is numbered on the outside to aid your record-keeping.
As with all our tickets, there is no extra charge for consecutive
numbering and up to five lines of crash imprinting. Stock
