MICHIGAN UNIFORM LAW CITATION (Form MI-17-6) is a six-part citation. (Price
Group 17) The face of all six copies is shown at right.
The backs of the first three copies ("Court" copies)
contain information on Court Findings, Hearing/Trial Dates
and Bond Information. The back of the fourth copy ("Police
/ Enforcement Agency / Prosecutor Copy") contains diagrams
for Officer's Notes and Case Disposition Information. The
back of the fifth copy ("Misdemeanor Copy") instructs
the offender on how to proceed. The back of the sixth copy
("Civil Infraction Copy") also instructs the offendor
on how to proceed.
The citation is 4.25 inches by 9 inches after being torn
at the perforation and is booked in sets of twenty-five. Each
book is numbered on the outside to aid you in your record
keeping. As with all our tickets, there is no extra charge
for consecutive numbering and up to five lines of crash imprinting.
