LOUISIANA "THREE VIOLATION" UNIFORM TRAFFIC
SUMMONS (Form LA-17-4DPS) is a four-part citation. (Price
Group 16) The face of all four copies is shown at right.
The back of the first copy (white "Complaint-Affidavit")
contains Abstract of Court Record information. The back of
the second copy (yellow "Abstract of Court Record for
State Licensing Authority of Conviction") contains Abstract
of Court Record information for the Driver's License Division.
The back of the third copy (pink "Police Record")
contains spaces for Report of Action on Case, Officer's Notes,
Witnesses, and Vehicle Defects. The back of the fourth copy
("Summons" hard copy) contains instructions for
the defendant, Guilty Plea Waiver, and a perforated Temporary
Operator's License.
The citation is 4.25 inches by 9.375 inches before being
torn at the perforation and is booked in sets of twenty-five.
