KANSAS "THREE VIOLATION" UNIFORM NOTICE TO APPEAR
AND COMPLAINT (Form KS-17-4DPS) is a four-part citation. (Price
Group 16) The face of all four copies is shown at right.
The backs of the first two copies ("Complaint"
and "Abstract of Court Record") contain the Court
Record and spaces to enter pleas, fines, etc. The back of
the third copy ("Police Record") contains information
for Officer's Notes. The back of the fourth copy ("Summons"
hard copy) contains waiver and plea instructions for the defendant.
The citation is 4 inches by 9.25 inches before being torn
at the perforations and is booked in sets of twenty-five.
Each book is numbered on the outside to aid you in record
keeping. As with all our tickets, there is no extra charge
for consecutive numbering and up to five lines of crash imprinting.
