Georgia Uniform Traffic Citation UTC Summons and Accusation
(Form GA-17-5R) is a five-part carbonless citation for Georgia Law Enforcement. (Price
Group 16G) The face of all five parts of the Georgia Traffic Citation is shown at right. Updated for 2010.
The back of the first three copies contain court docket information.
The back of the fourth copy ("Violator's Copy) contains
instructions for the violator to follow. The back of the fifth
copy (Officer's Copy") contains Officer's Notes for Testifying
in Court. There is no extra charge for consecutive
numbering and up to five lines of custom front side text. Stock
tickets are shipped within seven business days from
time the order date. We offer the Georgia Uniform Traffic Citation in runs as low as 100 tickets.
For orders under 5000 tickets, there is an additional charge to change or add text to the back of the violators copy. This charge is $115.00.
