Illinois Citation and Complaint (Form IL-17-6R) is a six-part carbonless citation for Illinois Law Enforcement.(Price Group 17) The face of all six copies is shown at right. Updated for 2010.

The back of the first copy (white "Complaint" copy) is blank. The back of the second copy (canary "Report of Conviction" copy) contains Court Action and Other Orders. The back of the third copy (white "Court Disposition" copy) contains more Court Orders. The back of the fourth copy (green "Police Record" copy) is blank for Officer's Notes. The back of the fifth copy (goldenrod "Violator's Copy") contains instructions for court appearance or payment of fines. The back of the sixth copy (blue "Court Communication Copy") contains more information on court appearances.

The back of the first copy (white "Complaint - Court Record Copy") contains spaces for bail, fines, and other court information. The back of the second copy (white "Disposition Report - Enforcement Agency Copy") contains Court Disposition information. The back of the third copy (pink "Police Record-Officer's Copy") contains the same information as the back of the first copy. The back of the fourth copy (goldenrod "Copy of Complaint & Summons - Violator's Copy") contains bail and waiver information.
