The PARK-3 (Price Group 16) is a three-part form, with the third part being a hard copy that converts to an envelope so the offender can easily make payment by mail. Our standard copy as it appears near the bottom of the ticket (the six lines just above the officer's signature) can be changed to include the amount of the fine and the number of hours.

The PARK-3 is our most popular ticket, but we also offer this form in a two-part without the envelope. Form PN-H (Price Group 11B) contains a soft top copy with a hard second copy. Form PN-1 (Price Group 6A) contains two soft copies.
