GEORGIA UNIFORM TRAFFIC CITATION, SUMMONS AND ACCUSATION
(Form GA-17-5R) is a five-part citation. (Price
Group 16G) The face of all five parts is shown at right.
NOTE: Make sure you have the new Georgia Citations. Look
for revisions number DPS-32(1/02) in the top left corner.
Usage is mandatory beginning July 1, 2002.
The back of the first three copies contain court docket information.
The back of the fourth copy ("Violator's Copy) contains
instructions for the violator to follow. The back of the fifth
copy (Officer's Copy") contains Officer's Notes for Testifying
in Court.
The citation is 5.625 inches by 8.5 inches after being torn
at the perforations and is booked in sets of twenty-five.
As with all our tickets, there is no extra charge for consecutive
numbering and up to five lines of crash imprinting. Stock
